Instructions: How to Submit Your Signature in Washington.

Step 1: DOWNLOAD FORM

Step 2: SIGN THE PETITION

Collect Signatures on the Form as Follows.

• There are 7 fields in each row:
  • Signature
  • First Name (print)
  • Last Name (print)
  • Date of Birth
  • Street Address
  • City
  • Zip Code
• Legibly complete every field. This is important because the Washington Secretary of State will be validating every signature to confirm each signer is a registered Washington State voter and that the voter has not signed a nominating petition for any other candidate.
• (optional) Collect signatures from others (print as many copies of the form as needed and mail them in when finished collecting or when a form is full)
• (optional) Confirm your own Washington voter information online by using your first name, last name, and date of birth at: https://voter.votewa.gov/portal2023/login.aspx
